Request for On-Site Technical Assistance

As a Partner in our program, you have access to a wealth of information and assistance. One of our most useful tools to assist you is by visiting your site and providing direct consultation and advisement. We will schedule a site visit based on the specific requests in the following form. Once submitted, you will be contacted by Sustainability Division staff to schedule your on-site assistance.

Note: If you are requesting a baseline assessment, you must attach 12 months of water & sewer, natural gas, electricity, and solid waste bills.

Note: You may check a maximum of two areas per request. If the option notes “include bills,” you must attach 12 months of bills to this questionnaire.
